IV: Fluids and Meds

Nursing & Emergency Medicine

A bad IV start:

  • Can result in a multiple stick event
  • By more than one healthcare provider
  • Over the ambulance ride in
  • Through the Emergency Dept. stay
  • To the floor, through a 1st and 2nd shift.

Then the next complication in this cascade of events is the  INFILTRATON - that leakage of meds/fluids out of the vascular system and into the subcutaneous tissue - presenting a whole new problem AND a delay in treatment.

What are the infiltration problems:

cellulitis, necrosis, septicemia, even amputation - markedly affecting medical outcome.

How does a delay in delivery of that infusate/biological affect the medical outcome?  Let's describe the IV administration of Pitocin during Labor & Delivery:  Pitocin increases uterine contractions, speeding up delivery.  But if the Pitocin is infiltrating into the subcutaneous tissue

  1. the Pitocin is not being delivered to the uterus.
  2. this will impair the effort to increase contractions and speed delivery.

How do we avoid all of that?
